Description
'Geographical pathology return of peptic ulcers (duodenal and gastric), gastric carcinoma and gallstones': these questionnaires are in the form of pre-addressed Air Mail letters to D. P. Burkitt MD FRCSE, Medical Research Council, 172 Tottenham Court Road, London W1P 9LG. The form is divided into two sections, the first section for 'Patients with peptic ulcers (cases confirmed by operation or x-ray)' contain columns for name, how diagnosed X or O (often also contains the date); age, sex, occupation, site of ulcer (gastric, duodenal or uncertain), perforation, stenosis, bleeding. The second section is for 'Patients with gastric carcinoma' containing columns for name, how diagnosed X or O, age, sex, occupation, biopsy yes/no, did biopsy confirm diagnosis? Form also contains name of hospital, country, ref. no., name of doctor and month of return, e.g. Dec 1974.
'Epidemiological survey of non-malignant / malignant diseases'. These are grouped by hospital and each hospital's monthly return sheets are filed chronologically. Some months are crossed through as a 'nil return' where no cases presented that month. On these returns one side of the page has a table for disease initial (appendicitis, gallstones or haemorrhoids or peptic ulcer), how diagnosed C, O or X, name of patient, age, sex, district of residence, tribe or caste, peptic ulcer type (gastric, duodenal or site uncertain) and if perforation, stenosis or bleeding. The second side of the form is for selected tumours with column for type of cancer (breast, Burkitt's lymphoma, oesophagus or stomach), name, age, sex, district of residence, tribe or caste, confirmed by biopsy yes/no.
The survey evidently covered Jan 1974-Dec 1975 according to the dated questionnaire returns. Some countries have returns on both forms and others just on one type of form but may cover both years.
