About this work

Description

This is a book of forms: one half of each form was sent out to the employer of the Retreat trained nurse, and the other half left as a stub copy in the book. Gives name and address of employer, name of nurse employed, dates and fees. Some of the reverse stub pages have been used to record further notes about private nursing accounts. The volume has lost its binder and the pages are now all loose.
