Segment 1 First, the abdomen is opened by longitudinal incision. A transverse incision is made in the utero-vesical fold. The bladder is separated by lifting it up with a clamp. The uterus is opened in its lowest portion by a sagittal incision, passing mainly through the portion of the cervix freed from the bladder. The baby is delivered by podalic extraction. The uterus is drawn out of the abdominal cavity. The placenta is removed using Crede's direct method. The edge of the wound is held with clamps. Segment 2 The uterine incision is closed partly by interrupted sutures and partly by continuous sutures, using catgut. The uterus is massaged whilst the suture is made. Segment 3 The sero-serous suture continues. The bladder is returned to its former place above the uterus, so that the uterine suture is completely hidden by the bladder. The uterus is dropped back into the abdominal cavity. Gynergen and Hypophysin are injected into the uterine tissues. The abdominal wound is sutured in layers by continuous catgut sutures.
